Existing to integrate primary care with secondary and community services. Combining of resources allows us to offer a wider range of services and a more holistic approach to our population’s care.
The network comprises four closely collaborating practices. Patients continue to access their GP practice and the services they offer as usual, however, the Primary Care Network provides an array of staff who work across all practices.
We want to improve your experience as a patient, making sure you are provided with the most appropriate treatment, in the best place, in a timely manner, improving the quality of care provided to patients and ensuring NHS resources are used appropriately.
