How Farnham PCN delivers Social Prescribing...
Referral is via the GP or other health or social professional. It offers a flexible approach with options that meet patients needs. Like engaging over the phone, meeting at Farnham Hospital or a local community space.
Individual’s are supported over time, with approx 6-12 contacts over a 3 month period, or longer if required.
