About Farnham Primary Care Network
Who are we?
Farnham Primary Care Network (PCN) is a group of practices working together to focus on local patient care. Our work is a collaboration with Downing Street Group Practice, Farnham Dene Medical Practice and Farnham Park Health Group. We provide a wider range of services to support our patients.
What does Farnham PCN and Farnham Integrated Care Services deliver?
The main purpose of our PCN is to enhance patient outcomes, improve the quality of care and increase the efficiency of healthcare delivery. As a PCN we achieve these goals through improved coordination of care, offering a wide range of services, enhancing access to care, managing population health, promoting preventive care, health education and prioritising a patient-centred approach.
Our ultimate aim is to deliver better health outcomes for the local population, as well as a reduction in health inequalities. We work along side Farnham Integrated care services which houses some of our clinicians, the same day service and enhanced access.
Farnham Integrated Care Service is a separate entity which is the vehicle for implementing locality plans and supporting high quality integrated care in Farnham.

Additional healthcare professionals are being recruited to work on behalf of Primary Care Networks. Roles include Pharmacy Technicians, Advance Paramedic Practitioners, Health Care Assistants and GP’s to name a few.
By introducing a greater skill mix, local people will get more access to the support they need to keep people healthy and independent.
