What is Home Visiting Service?

Following triage by the Practice GP, the Home Visiting Paramedics carry out visits to housebound patients to continue this assesment (for those either permanently or temporarily housebound due to ill health).

How Home Visiting Service can help you...

  • Timely medical care
  • Reduced hospital admissions
  • Post-discharge support
  • Supporting GP’s with timely housebound Care
  • Integration with healthcare system

By providing timely intervention and medical care at home, home visiting paramedics can help prevent unnecessary hospital admissions. They may also follow up with patients after hospital discharges, ensuring a safe discharge and that the patient has fully understood their post-treatment care plans, preventing readmission. The Home Visiting Paramedics work in collaboration with Secondary Care, other Primary Care providers, and wider healthcare professionals, ensuring seamless care transitions and continuity of care.

Mrs B,

Patient

''I just want to say a huge thank you to the social prescribing team who called me after a home visit from the GP. I have had to try and organise a carer for my In-Laws urgently and did not have a clue how difficult it would be. Suzi chatted through the types of care and the referral options in adult social care and guided me through the options. I received a prompt email providing links to PA providers. Great support from a knowledgeable team. Thank you.''

How Farnham PCN delivers Home Visiting Service...

As directed by a GP, the HVS team conduct thorough assessments of patients’ symptoms and health conditions.

Paramedics address any immediate health concerns the patient may have, discuss their findings and organise a treatment plan with the relevant clinician.

They are also provided with the time to apply a holistic approach whilst evaluating the patient, considering their health and wellbeing and environment that may be contributing factors. This may support any reasoning for proposing an onward referral to services such as Social services, Occupational Therapy, Diagnostic & Treatment Centre, District Nurses, Continence Team, Rapid Response, The Virtual Frailty Ward and other relevant Community Services.
