As directed by a GP, the HVS team conduct thorough assessments of patients’ symptoms and health conditions.
Paramedics address any immediate health concerns the patient may have, discuss their findings and organise a treatment plan with the relevant clinician.
They are also provided with the time to apply a holistic approach whilst evaluating the patient, considering their health and wellbeing and environment that may be contributing factors. This may support any reasoning for proposing an onward referral to services such as Social services, Occupational Therapy, Diagnostic & Treatment Centre, District Nurses, Continence Team, Rapid Response, The Virtual Frailty Ward and other relevant Community Services.
